Tangible Request Form (Welcome Packs)

We do our best to honor all tangible requests with our Welcome Pack Program for licensed foster and kinship families bringing in emergency placements working with DSS and reunifying families with active DSS cases. Welcome Packs include the following essential items: 5-7 outfits Pajamas Socks and underwear Blanket and or lovie Bottles, sippy cup, water bottle (depending on age) pacifier, diapers, wipes, Hygiene items Small toy or book Reunifying Families will find additional tangible options in your dropdown, such as: Toddler beds Backpacks Carseats Grocery gift card 

After submitting the form below, you can expect: Confirmation communication within 24 hours to set up a time to deliver a Welcome Pack and assist with connections and other necessary resources. One of our volunteers will deliver Welcome Pack(s) to your home if you are within 20 miles of our Resource Center, 4919 Monroe Road. If you are outside our delivery range, you may pick up Welcome Pack(s) within 2 days of submitting a request at the above address.
